Immunizations

The following information is summarized from the WA Dept of Health School and Childcare Immunization webpage. Please refer to this webpage or speak with your school nurse if you have any further questions. Parents can also access/print their student's immunizations by signing up with MyIRMobile.

Before a child may attend a school or child care center, a parent or guardian must provide proof of the required immunizations or immunity using a Certificate of Immunization Status (CIS) form. The CIS may be printed from the WA Immunization Information System (IIS). Parents can also print a CIS by signing up with MyIRMobile. If the hardcopy form is used, it must be medically verified by accuracy with either a health care provider's signature or by attaching medical immunization records to the completed CIS so school or child care staff can verify it for accuracy. English (PDF) | Español (PDF)  More languages are available on the WA DOH webpage.  

The Mukilteo School District uses the Washington State Immunization Information System (WAIIS) School Module online system to monitor immunization compliance for our students.  You will be asked to provide consent for us to maintain your student's immunization record at time of enrollment and doing so will result in more complete and accurate immunization records for our students.

Students entering school (preschool - grade 12) must be in compliance with required vaccinations before they may attend public school according to Washington state law. The requirements and rules vary depending on the student's grade. Conditional Status Attendance

Children who are in the process of completing their required immunizations may remain in school or child care in a temporary conditional status.

Students Out-of-Compliance with Immunization Requirements

By law (RCW 28A.210.120) it is the duty of the administrator of every public and private school and day care center to prohibit the further presence at the school or day care center for any and all purposes of each child for whom proof of immunization, certification of exemption, or proof of compliance with an approved schedule of immunization has not been provided in accordance with RCW 28A.210.080 and to continue to prohibit the child's presence until such proof of immunization, certification of exemption, or approved schedule has been provided. Students lacking immunization records who are covered under the federal McKinney-Vento Act may not be excluded.

Exemptions

Washington State Law allows parents or guardians to exempt their child from school immunization requirements. Exemptions may be claimed for personal/philosophical, religious or medical reasons. Measles, mumps, and rubella may not be exempted for personal/philosophical reasons. To request an exemption, a completed Certificate of Exemption must be submitted to the school or child care center. A copy of the form and directions for its completion can be found on the Washington State Department of Health School and Child Care Immunization webpage. Please note that a medical provider's signature is required in most cases. Students with exemptions on file may still be required to stay home from school if there is an outbreak in their school.
