Below are many commonly used forms in the Mukilteo School District.

TRANSLATION - For online translation into another language, for any of the forms below: Copy the correct form link and paste it into Google Translate. Select the language needed and click the new link created by Google Translate to open and complete the form in that language.

For the transfer forms: a transfer request is NOT needed if you have moved into the Mukilteo School District and want to register your student(s) at the school assigned to your address. If you have moved and need us to change your student's school of registration, please make sure your student's current school has your new address and then contact the new school to change enrollment. If your student(s) is/are not currently attending a school in the Mukilteo School District and you need to register them, check the school attendance boundaries for your new address and visit our registration page to register your student(s). If you do need a transfer form, they are linked below.

In-District Transfer Requests

We begin accepting requests for the upcoming school year the first student day in January at 8am PST.

Decisions on transfers are made in September, through the 7th day of school.

Applications for transfers for the current school year will be accepted through March 31 of each school year. No new transfer requests for the current school year will be accepted after that date unless there is a recent change in the resident student's address.

Out-of-District Choice Transfer Requests

For the upcoming school year:

New requests (including grade breaks going from Pathfinder to 1st grade, 5th to 6th grade or 8th to 9th grade) we begin accepting requests August 1st.

Returning (or current students, not at a grade break) AND employee requests, we begin accepting March 1st.

Per OSPI guidelines, the district has 45 days once we receive the transfer request to respond.

Choice Transfer Requests received after the first day of the current school year will be reviewed on a monthly basis. All Choice Transfer Requests must be acted upon by accepting or denying the request within 45 calendar days of receipt by the District.

  • Choice Transfer Request Portal 

    Choice Transfers have to be submitted/resubmitted annually. In March, make sure you've submitted requests for your students for the upcoming fall!

    The Choice Transfer Request Portal is an online form submission tool used by most school districts within Washington State. A Choice Transfer request needs to be made when a student wants to attend a school outside of the school district where the student resides/lives, even if they are already attending the school. The parent/guardian shall request the release of their child by completing a Choice Transfer Request through the Choice Transfer Request Portal. Once the Choice Transfer Request is submitted by the parent/guardian, the resident district will determine whether or not to release the student. If it is determined to release the student, the resident district will submit the request to the requested district. Then it is up to the requested district to determine acceptance or denial to the school requested. As each of these steps are completed, the parent/guardian will receive e-mails to the e-mail address provided in the request. If you are unable to submit the request through the online portal, please contact your resident district. If Mukilteo School District is your resident district, the Choice Transfer contact for grades 6-12 is Brenna Fullner and for grades K-5 is Judy Heminger.

    If you have moved into a new school district and want to enroll at your neighborhood school, a Choice Transfer is not needed.

  • An In-District Transfer Request is needed when you live in the Mukilteo School District, and would like your student(s) to attend a school that is not your neighborhood school. It is also used when your student is already attending school, and you move during the school year within the district, but outside of the service area for your student's current school, and would like to request for your student(s) to remain at their current school. If you are not sure what your neighborhood school is, we have the district map and Versatrans where you can see which school your student(s) should attend based on your home address. If you have any questions, please contact Judy Heminger for grades K-5, or Brenna Fullner for grades 6-12.

    Applications for transfers for the current school year will be accepted through March 31 of each school year. No new transfer requests for the current school year will be accepted after that date unless there is a recent change in the resident student's address and you need to submit a "Request to Remain"A Request to Remain is when you have moved outside the boundaries of the school that your student(s) is/are currently attending and would like to submit a request for them to continue to attend at their current school. 

    • If you are new to the district and need to enroll your student(s), please enroll them at your neighborhood school. You do NOT need to fill out a transfer request to enroll at your neighborhood school.
    • If you move during the school year and would like to change your student's enrollment to your new neighborhood school, please contact your new neighborhood school and they will help change your student's enrollment. You do NOT need to fill out a transfer request to enroll at your neighborhood school.

    Transfers in or out of ACES in-person or the ALE online program, are processed through the school. Please contact their office at 425-366-3900 to make a request.

    Once approved, an In-District Transfer is granted through the highest grade offered at the school the transfer is approved for and does not need to be renewed while your student attends that school.
    Elementary school: grades K or 1 - 5
    Middle school: grades 6 - 8
    High school: grades 9 - 12
    PKC: kindergarten only

  • For all grade levels, families should not schedule vacations or travel while school is in session. If a family vacation or travel must occur while school is in session, it must be prearranged. Pursuant to district Procedures 3122P, the Principal (or designee) may excuse up to five (5) school days for a pre-arranged absence, per student each school year. Please use one of the forms below to meet with your school principal or designee to discuss a pre-arranged absence from school.

  • This form is used to report harassment, intimidation or bullying (HIB) to the school (counselor, Principal, Assistant Principal) or to the Student Services Department at the district. It may be filled and submitted by a student or a parent/guardian. Visit the Student Services page for additional information.

  • This form is used for students ages 8-18 to notify the student's resident school district that they will be homeschooled for that school year. Once completed, forms can be emailed or mailed to Brenna Fullner in our Student Services Department at our district office located at 9401 Sharon Drive, Everett, WA 98204. This form is not used if your student ages 8-18 will be enrolled in an online school like WA Virtual Academy (WAVA), Insight School of WA or Connections Academy. For those schools, you need to submit a Choice Transfer Request at the top of this page.

    Homeschool Intent Form

  • This form is used for students ages 2-7 only, that are currently enrolled in school, and will be withdrawn to homeschool. Once completed, forms can be emailed or mailed to Brenna Fullner in our Student Services Department at our district office located at 9401 Sharon Drive, Everett, WA 98204.If a student is under the age of 8 and not yet enrolled in public school, a homeschool form does not need to be filled and submitted. This form is not used if your student ages 2-7 will be enrolled in an online school like WA Virtual Academy (WAVA), Insight School of WA or Connections Academy. For those schools, you need to submit a Choice Transfer Request at the top of this page.

    Homeschool ages 2-7 Withdraw Form

  • This form is used for proof of address when enrolling a student, or to keep a student enrolled, when a student AND their family (legal parent(s)/ guardian(s)) are all living with or renting from another family/adult, and do not have any approved proof of residence in their name. This form is NOT used for families/students that qualify for McKinney Vento/Homeless assistance. Full instructions are on the first page of the document.

  • This form is used for proof of address for enrolling a student, or keeping a student enrolled, when the student does not live with their legal parent/guardian. This form is NOT used for families/students that qualify for Unaccompanied Youth or McKinney Vento/Homeless assistance. Full instructions are on the first page of the document.
